Survey Tool Recommendation

We are looking to implement a web-based survey option for our end users once a ticket has closed. Can anyone recommend a survey tool / product that integrates well with the Service Desk?

There is a company in the US called HDI (www.thinkhdi.com) that has a service called CSI that will send an email to your users based on whatever database rules you setup. The way it works is you send your SC information to the CSI tool and then it will send an email to your end user to gather their feedback.
