Hiding time zone and language choice in ssp

How can I hide choice about time zone and language when ssp user log in to self service pages in service desk 4.5.
And how can I add additional fields to ssp form, for example: service, CI or something else. I think that can do by setting reguired fields. The service is shown an form but I can't list my services... Can I do it by relating SLA (where in receivers section I releate my user organization) with Services.

If you do an advanced search on the OV IT Service Mgmt and Service Desk area, with the keywords 'service', 'pages', 'time' and 'zone', you will find more results.
