Feed back form to Application user

Hi all,

Servicedesk 4.5 with sp 20

When ever any servicecall raised by any sp account user, there is an option to the user to give feed back after resolving the call through ServicePage.
How i can i give the same option to Application user.
I am able to give only two views to application user i.e Servicecalls assigned to me(Restrickted list) and servicecalls assigned to my work group(full list).

Please let me know how i can give the view CALLS OPEN BY THE APPLICATION USER.
