Error when creating database with database configuration wizard

I've installed a Windows 2000 sp4 advanced server with SQL 2000.

after installing jvm and servicesdesk 4.5 from the installation CD I try to configure the database using the database configuration wizard using the option create a new database and user or use a already created database and user I get the error:

failed to open connection to service desk repository java.io.InterruptedIOException:
receive timed out

After clicking ok the following message appears;

this user either doesn't exist or is not an administrator

When creating a new database and user I use the following credentials:

username = sa
password = configured paasword
host= ip address of server
port=1433
instance=SQL server name (not W2K server ID)

I have one HP service desk service running (Started)in my services screen

Any ideas?

SQL server hostname (= w2k hostname) plus port should redirect you correctly to sql server instance. Don't specify the field instance

Thanks Jasper for the information.

I tried you suggestion;

user id=sa
password=
host=name of W2K server
port 1433
instance="left blank"

now I get the error

Failed to open connection to service desk repository

Login failed for user sa; reason: not associated wirh a trusted SQL connection

I checked my services window to see if the HP service desk service was still running, I can't get it to start and remain started today.

Hi, please make sure your MS SQL Server has both Windows and SQL authentication turned on. SQL Server by default only has Windows authentication turned on.

It will not work unless this is set properly.

I did all that rules with making the sql and windows authentication to sql server and still give me the error:
" Faild to open service desk repository.
connection refused :connect."

My problem when configure database after I click Finish to create:
java.sql.SQLException: Invalid object name 'MASTER..SYSLOGINS'

What is this problem? I use W2k with SQL2k SP3.
