Creating Service Call when incident is raised.

Hi,
I need to create a service call in OVSD whenever an incident is created in OVSD.For achieving this functionality , we are planning to write a smart action for incident call.When the user selects this smart action from incident menu, the service call form(with values from incident) should get displayed.

Can anyone please let me know how to use the sd_dataform command with values from incident?
