Can't create e-mail based Service Calls

I have about 6 different e-mail based service call setups. I recently upgraded from OVSD 4.5 SP11 to SP17. I tried to setup a new e-mail based integration and it isn't working. This is the error I get.

"The following error(s) occurred:
---------------------------------------------------
For service call the following fields are required:

Alternate Phone,
Category,
Status.
---------------------------------------------------"

I have deleted and re-created two seperate templates and nothing will work. I know the templates are populated correctly, my other e-mail integrations are working fine. I just can't seem to get any new ones to work. The integration worked in my development environment, it was created pre service pack however.

Thoughts?

Oracle 9.2.0.5
OVSD 4.5 SP17
