Thanks for responding Alex. I actually did begin a more in depth review of the logs after I posted. Turns out that in the process of changing the workflow and then updating my DEV instance to reflect what we have in TEST, I did not migrate the Project workflow BACK to the DEV environment. This, coupled with a request type field change resulted in the app not being able to index the records properly. After I migrated the WF back and changed the RT, everything got back to normal.
